Waiver of Liability
I, [Name of parent or guardian], am the parent or legal guardian of [Name of child]. I am aware that my child will be participating in the Kids in Business Market Stalls on 2nd September 2023 [Date of event] at Carysfield Reserve Bass Hill [Location of event].
I understand that there is a risk of injury or loss associated with participating in this event. I hereby waive any and all claims against Kids in Business, its employees, agents, and volunteers for any injury or loss that may occur to my child while participating in this event.
I understand that I am responsible for my child's safety at all times. I will remain with my child and monitor them at all times while they are participating in the market stalls.
I have read this waiver and understand its terms. I agree to be bound by its terms.
Additional Information
The organisers of the Kids in Business Market Stalls are insured. However, this insurance does not cover all risks.
Parents of stall holders under the age of 18 are required to remain with their children and monitor them at all times.
Kids in Business is not responsible for any child or stall or operator and will not be held liable to any harm, injury or loss that may occur.
By participating in the Kids in Business Market Stalls, you agree to the terms of this waiver and submitting this form is determined to be your digital signature of acceptance of this waiver.
